A personal check, cashier's check, or money order is required for each passport application.


Applicant Requirements for New Passports

  1. A valid picture identification in the form of either a driver’s license, state-issued identification card, or military ID, as well as a copy of the identification being relied upon.
  2. One of the following:
    • Original or certified copy of birth certificate from the state board of health with a seal. Birth certificates may be obtained from the state board of health, division of vital statistics, for each state. To contact the division for your state of birth, visit the Centers for Disease Control and Prevention (CDC) website.
      • Note: A certified birth certificate has a registrar’s raised, embossed, impressed, or multicolored seal, registrar’s signature, and the date the certificate was filed with the registrar’s office, which must be within one year of the applicant’s birth. Some short abstract versions of birth certificates may not be acceptable for passport purposes. All birth certificates must include the full names of the applicant’s parent(s).
    • Old passport: An applicant who submits an expired U.S. passport or passport card as citizenship evidence is not required to submit a photocopy.
    • Naturalization papers
      • Note: Applicants applying for a passport using Form DS-11 must provide a clear copy of their citizenship evidence in addition to the original or certified copy of U.S. citizenship. If writing is on both sides of the citizenship evidence, copies must be made of the front and back of the document. This is for primary U.S. citizenship evidence only, such as certified birth certificate, naturalization certificate, consular report of birth abroad, or certificate of citizenship.
    • Photocopies submitted:
      • Must be legible.
      • Must be on white 8.5-by-11-inch standard paper.
      • Must be copied using black ink.
      • Cannot be double-sided.
    • Applicants who fail to provide a photocopy of their driver’s license and/or citizenship evidence will be charged $0.15 per copy to complete their application.
  3. One 2-by-2-inch regulation passport photograph. Passport photographs are available at the Clerk & Comptroller’s Office for a fee of $16.

Note: Children must be present when applying for a passport. Both parents must sign for children 15 and under and present a long-form birth certificate with both parents’ names. If one parent is unable to be present, Affidavit DS-3053 must be provided along with a front/back photocopy of the nonapplicant parent’s ID.

Note: The birth certificate will be mailed, along with the passport application, to the U.S. Department of State. After processing, the birth certificate will be returned separately by mail.

Passport Renewals

If a passport has expired, or is about to expire, it may be able to be renewed by mail.

Adult passports issued within the last 15 years may be renewed by mail.

Expiring passports issued to children under the age of 16 will be treated as a new passport and a passport application is required.
